Best Practices Award Submission

Five companies will be selected to win for each of the International Testing Certification Super Weeks listed below
Only applications using this form will be considered
Each winning company will be awarded five days of free training at the International Testing Certification Super Week applied for. The five days of free training will be awarded to 5 members of the test team
Forms must be submitted no later than:

February 15, 2008 to be considered for the Chicago Super Week (Winners have been announced)
October 2, 2008 to be considered for the Las Vegas Super Week

Wining companies will be announced approximately three weeks before the International Testing Certification Super Week applied for
Winning companies will be featured during the International Testing Certification Super Week applied for. One winning company will be featured each day.
The Award does not include airfare or hotel accommodations
All information submitted by winning companies will be published on the IIST web site
By applying for this award, you are granting IIST the right to publish information you provide on its web site and make it available to other software test professionals
